Cortisol, Stress, and Why You're Gaining Weight Around Your Middle
There's a particular kind of frustration I hear from women regularly. They're eating well. They're exercising. Nothing particularly dramatic has changed in their diet or routine. And yet the weight is shifting. Specifically, to their middle. Belly fat that wasn't there a few years ago, or that's become noticeably harder to shift.
The standard response from healthcare is to recommend eating less and moving more. And I understand why: it's the framework that works for a lot of people. But for women in their late thirties and forties, particularly those carrying significant professional or personal stress, that advice frequently misses the actual driver.
The driver is often cortisol.

What cortisol is actually doing
Cortisol is your primary stress hormone. It's produced by the adrenal glands in response to physical or psychological stress, and it's essential: without cortisol, you couldn't mount an immune response, regulate blood sugar, or get out of bed in the morning. The problem isn't cortisol itself. It's chronically elevated cortisol in a body that doesn't have the biological opportunity to fully recover.
Here's the mechanism that most people don't know: cortisol has a direct, targeted effect on visceral fat (the fat stored around your internal organs). It stimulates an enzyme called lipoprotein lipase specifically in visceral adipocytes (the fat cells surrounding your organs). This is a direct biochemical pathway: chronic cortisol elevation actively directs fat storage to the abdominal region, independently of calorie intake.
Cortisol also promotes gluconeogenesis, a process where your liver produces glucose from non-carbohydrate sources. This raises blood glucose levels, which raises insulin, which promotes fat storage. The combination of elevated cortisol and elevated insulin creates a particularly efficient fat-storage environment, especially in the abdominal area.
This is why you can be eating a reasonable diet and still gaining weight around your middle. The mechanism isn't caloric; it's hormonal.
How chronic stress shifts your hormonal environment
Beyond the direct fat storage effect, chronic stress has downstream consequences for the entire hormonal picture.
Research suggests women with high levels of perceived chronic stress show measurably elevated cortisol awakening responses (the surge of cortisol that occurs in the first 30 to 45 minutes after waking). An elevated cortisol awakening response is associated with disrupted sleep architecture, impaired immune function, and, in the luteal phase, lower progesterone-to-oestrogen ratios.
That last point matters. When cortisol is chronically elevated, the body preferentially produces cortisol over progesterone, because both hormones share the same precursor (pregnenolone). This is sometimes called "pregnenolone steal", though the term is somewhat simplified. The practical result: chronic stress can reduce progesterone output in the second half of the cycle, worsening PMS, disrupting sleep, and contributing to oestrogen dominance patterns.
This is not just a psychological experience. Chronic stress changes the hormonal environment in measurable, documented ways.
Why high-achieving women are particularly vulnerable
Professional women often carry a specific type of stress load: cognitively demanding work, responsibility for others (at work and at home), inconsistent eating and sleep patterns, and a cultural expectation to manage all of this without visibly struggling.
The body doesn't distinguish between "important" stress and "background" stress. It responds to chronically elevated demand in the same way: prolonged cortisol elevation, suppressed digestion and immune function, disrupted sleep, and accelerated visceral fat accumulation.
What I see in practice (and what I experience myself) is that the women who are most on top of everything externally are often the ones most physiologically depleted. The efficiency comes at a cost that shows up in the body.
The weekly stress reassessment: why it's physiologically relevant, not just a mindset exercise
One of the habits in the 90 Hard Women's Health programme is a weekly stress reassessment: write your top three stressors, rate each as controllable or not controllable, and commit to one action for each controllable one.
This sounds like soft self-help. The neuroscience is more interesting than that.
Naming and externalising a stressor reduces amygdala reactivity. The amygdala is the brain's threat-detection centre; it drives the cortisol response. Writing down a stressor activates the prefrontal cortex, the part of the brain responsible for rational assessment and regulation. Scheduled action reduces the "open loop" that the brain keeps returning to at night. Unscheduled, unresolved stressors create a continuous low-level threat signal that keeps cortisol elevated.
A 10-minute weekly review is a physiological intervention. It's not the whole solution, but it's a measurable one.

What actually helps: practical anti-cortisol strategies
1. Zone 2 cardio over high-intensity exercise when stress is elevated. High-intensity training is an additional cortisol stressor. If you're already running on a chronically elevated cortisol baseline, adding HIIT three times a week can compound the problem. Walking, swimming, or cycling at a conversational pace for 30 to 45 minutes is physiologically very different: it lowers cortisol rather than raising it.
2. Consistent sleep and wake times. The cortisol awakening response is regulated in part by your circadian rhythm. Irregular sleep patterns disrupt this rhythm and amplify the cortisol spike on waking. Consistency (within 30 minutes of the same time each day) is one of the most evidence-based interventions for cortisol regulation.
3. Anti-inflammatory eating. Chronic inflammation and chronic cortisol are bidirectional: each drives the other. An anti-inflammatory dietary pattern (adequate protein, omega-3 fatty acids, polyphenol-rich foods, reduced ultra-processed food) directly reduces the inflammatory burden that stresses the HPA axis. This is why dietary change is part of the picture even when the primary driver is stress rather than food itself.
4. The bedtime brain dump. Writing down tomorrow's tasks before bed reduces the "open loop" effect on the prefrontal cortex and decreases sleep-onset time. A 2018 Baylor University study found 5 minutes of written task planning before bed reduced sleep onset by approximately 9 minutes. Better sleep = lower cortisol awakening response the next morning.
5. Address the root cause. Supplements and lifestyle habits help, but if your stress load is genuinely unsustainable, the only real solution is changing the load. This is the conversation I have in coaching sessions more than any other: not what to take for cortisol, but what to stop doing, delegate, or let go of.
