Tired before the day has started. Strength going, belly growing, drive gone.

You put it down to work, to age, to sleep. Somebody told you your levels were "normal". You still don't feel like yourself.

Testosterone isn't only a male hormone, and it isn't only about sex. In men and in women it plays a part in energy, muscle, joints, mood, focus and libido. When it's low, many people feel it.

Is this you?

Nod at a few of these and it's worth a conversation.

Low energy

Tired all the time, even after a night's sleep.

Low libido

Desire has dropped, or gone.

Brain fog

Focus and memory aren't what they were.

Low mood

Flat, irritable or unmotivated, without a clear reason.

Muscle and strength

Harder to build, quicker to lose, even when you train.

Aches and joints

Joints and muscles ache more than they used to.

For men: the protocol

Diagnosis needs your symptoms and your blood results together, never a number alone.

1

Talk

The TRT Consultation, 45 minutes online. Your symptoms, history, medicines, sleep, and whether you want children now or later.

2

Test

Testosterone on 2 mornings, between 7 and 11am, about 4 weeks apart, plus safety bloods: full blood count, PSA, liver, cholesterol and blood sugar.

3

Decide and start

A 25-minute TRT Review to go through your results and decide together. If testosterone is right, it starts from there.

4

Review

Checks at 6 weeks, 3, 6 and 12 months, then yearly: how you feel, your level, your blood count and your PSA.

Through the skin

A daily gel is the usual start. It gives steady levels, can be stopped quickly if a blood result needs it, and works fully online.

Injections

A long-acting injection every few months is another option. We talk through which suits you.

Children in the future?

Testosterone lowers sperm production. If you want children now or soon, there are options that raise your own testosterone instead, and we go through them first.

The causes alongside

Weight, sleep, sleep apnoea, alcohol and some medicines all lower testosterone. We work on those alongside treatment, not instead of it when your level is clearly low.

For women: the protocol

For perimenopause and menopause, natural or after surgery. Your level falls with age, and drops sharply if your ovaries are removed.

1

Talk

The Menopause Consultation, or the TRT Consultation if testosterone is your only question, 45 minutes online. We ask about libido, energy, mood, focus, muscles and joints from the first appointment. You don't wait months for testosterone to be discussed.

2

Test

A testosterone level before you start, to make sure it isn't already high, with SHBG to help read it.

3

Start

A small daily amount through the skin, at a woman's dose, often alongside body-identical oestrogen and progesterone.

4

Review

A level a few weeks after starting, then at your reviews. The dose follows how you feel first, and your level stays within the female range.

What it can help

Libido is where most of the research is. In my practice it can also make a real difference to energy, muscle strength, joint aches, focus, brain fog and low mood.

Side effects

At a woman's dose they are uncommon: extra hair, spots or a little weight gain. If they happen, the dose comes down. A deeper voice is rare, and if it starts, testosterone stops straight away.

Part of your whole plan

Testosterone is 1 of 4 body-identical hormones. Your plan might use 1, 2, 3 or all 4. See the Menopause Service

Is it working?

For women, we judge it by 3 to 6 months. If it isn't helping, we stop it, and look again at the rest of your plan.

Blood tests

If you already have recent results, bring them. If you don't, these are the ones that help. Men also need a PSA and a full blood count before starting, and we arrange those with you.

Questions people ask me

About the appointment
Is this for men or women?

Both. Men and women each have their own protocol: different tests, a different dose and a different target level.

I'm a woman in perimenopause. Should I book this or the Menopause Consultation?

Book the Menopause Consultation. Testosterone is part of it, alongside oestrogen and progesterone. If testosterone is your only question, for example you already take HRT elsewhere, the TRT Consultation is for you too.

Is it video or phone?

Either. You choose video or voice call, from anywhere in the UK.

Do I need a referral?

No. You book online yourself.

Will my GP know?

If your GP needs to know about your treatment, I write to them, so your records match.

Blood tests
Do I need a blood test before I book?

No. Book the consultation first, and we'll decide which tests you need. If you already have results, bring them, normal ones too.

Why do men need 2 morning tests?

Testosterone is highest in the morning and varies from day to day. 2 samples between 7 and 11am, about 4 weeks apart, show whether it is really low.

My level came back "normal". Can you still help?

Possibly. For men, a level near the lower end is read with SHBG and your symptoms, not on its own. For women, the decision rests on your symptoms, and the blood test makes sure your level starts and stays in the female range.

Treatment
Is testosterone only for libido?

No. In men, the research covers sexual function, mood, energy, muscle and bone. In women, libido is where most of the research is, and in my practice testosterone can also help with energy, muscle strength, joint aches, focus, brain fog and low mood.

How long until I notice a difference?

For women, we judge it at 3 to 6 months, and if it isn't helping, we stop and look again. For men, we give it at least 6 months, and the full benefit often takes up to a year.

Gel or injection?

For men, a daily gel is the usual start because levels stay steady and it can be stopped quickly. A long-acting injection is another option. For women, it's always through the skin, never by mouth.

Will it affect my fertility?

For men, yes: testosterone lowers sperm production. If you want children now or soon, tell us, and we go through the options that raise your own testosterone instead.

As a woman, will testosterone make me look masculine?

At a woman's dose, with your level kept in the female range, this is rare. The usual side effects are extra hair, spots or a little weight gain, and the dose comes down if they happen. A deeper voice can be permanent, so if it starts, testosterone stops straight away.

Is it safe for my heart?

For men with low testosterone, a large trial found no rise in heart attacks or strokes on treatment, but an irregular heartbeat (atrial fibrillation) and blood clots in the lung were more common. That is why your history is checked first, and your blood count and PSA are monitored throughout. For women, short-term heart data at a woman's dose are reassuring.

Can someone else be affected by my gel?

Yes, by skin contact. Let it dry, cover the area, wash your hands, and wash the area before close skin contact. If a child develops body hair, a growth spurt or genital changes, or a woman develops new hair growth or a deeper voice, get them checked the same day through your GP or 111, and tell the clinic.

Medicines, delivery and urgent symptoms
How do I get my medicine?

If we agree on a treatment, it can be posted to your home through Healistic, our home delivery partner.

How do I get repeat prescriptions?

Men on testosterone from the clinic request a repeat online. Women use the HRT repeat request. Reviews stay on their schedule either way.

What symptoms need checking urgently?

A lump in a testicle or breast needs an urgent referral through your GP, within 2 weeks: ask for it by name. Blood in your urine or semen needs checking urgently. If you're under the clinic's care, the clinic writes to your GP the same day. Chest pain, sudden breathlessness or signs of a stroke: call 999.
