GLP-1 Medications: What Every Woman Should Know Before Starting

There is no class of medication generating more noise right now than GLP-1 receptor agonists.

Some of that noise is warranted. The evidence for their benefits, especially in the area of metabolic health, is genuinely significant. Some of it is hype. And a lot of it is missing the context that determines whether these medications actually help the women taking them.

I want to give you that context.

What GLP-1 receptor agonists are

GLP-1 stands for glucagon-like peptide 1. It's a hormone produced naturally in the gut in response to food. It does several things: it stimulates insulin release from the pancreas, suppresses glucagon (which would otherwise raise blood sugar), slows the rate at which food moves through the stomach, and sends satiety signals to the brain.

GLP-1 receptor agonists are a class of medications that mimic or enhance the action of this hormone. They were originally developed for the management of type 2 diabetes, based on the well-established link between GLP-1 and blood sugar regulation. More recently, some have received licensing for weight management, based on the appetite-reducing and metabolic effects.

This post is educational. I'm not going to discuss specific product names. The medication landscape changes, prescribing decisions are individual, and any discussion of specific branded products falls outside what I can appropriately address in a public blog post.

What I can discuss is the mechanism, the evidence, and the context that makes the difference between these medications working well and not working at all.

What they actually do

They slow gastric emptying. When food moves more slowly from the stomach into the small intestine, the blood sugar rise after eating is more gradual. This reduces the insulin spike, and the subsequent blood sugar dip, that drives energy crashes and cravings.

They reduce appetite signalling in the brain. Many women who take GLP-1 medications describe a quietening of food noise: the persistent background mental preoccupation with food, hunger, and what to eat next. For some women, this is the most meaningful effect. The constant negotiation with appetite that they've been managing for years simply decreases.

They improve insulin sensitivity. This is a metabolic benefit that extends beyond weight loss. Improved insulin sensitivity has downstream effects on energy, hormone regulation, inflammation, and cardiovascular risk.

They have cardiovascular and metabolic benefits. The evidence here is now substantial. Several large clinical trials have shown reductions in major cardiovascular events in people with metabolic risk factors. This is not primarily a weight loss story. It's a metabolic story.

What they don't do

They don't work in isolation. This is the part of the conversation that's most often missing.

GLP-1 medications are most effective when combined with meaningful changes to nutrition, movement, and the lifestyle factors that drive insulin resistance and metabolic dysfunction in the first place. Without those changes, weight loss is typically regained when the medication is stopped. The underlying metabolic patterns remain unaddressed.

This is not a criticism of the medications. It's a description of how metabolic health actually works. No single intervention, whether it's a medication, a diet, or a supplement, overrides the fundamental importance of the full picture.

They don't suit everyone. There are contraindications, including personal or family history of certain thyroid conditions. There are side effects, predominantly gastrointestinal in the early weeks of treatment, that are significant for some people and minimal for others. There are cases where the medication is started and discontinued quickly because the side effect burden isn't manageable.

The decision to start a GLP-1 medication should be made with a prescriber who has reviewed your full medical history, your current medications, and your individual risk factors.

They're not a shortcut. I say this not to moralise but because the expectation that they function as a shortcut is the most common reason they disappoint. Women who start GLP-1 medications expecting them to do all of the work often find they lose weight initially, don't address the habits or metabolic patterns driving the issue, and find themselves in a difficult position when they want to stop.

The missing piece: integrated support

The women I see in my clinic who have the best outcomes with GLP-1 medications are the ones who also have support around nutrition, movement, and the other lifestyle factors that affect their metabolic health.

Not because the medication doesn't work without that support. It often does, in terms of weight loss numbers. But because sustainable metabolic health requires understanding what your body needs, not just reducing appetite pharmacologically.

What does integrated support look like? It looks like a nutrition framework that's adjusted to work alongside the medication. It looks like movement guidance that accounts for the muscle mass loss that can accompany significant weight loss, because muscle is metabolically critical and worth protecting. It looks like monitoring: blood tests, regular check-ins, and adjustments based on how you're actually responding.

It looks like someone who is thinking about your whole metabolic picture, not just a number on a scale.

A note for women specifically

The relationship between GLP-1 medications and female hormonal health is an area where the evidence is still developing.

What we do know is that metabolic health and hormonal health are deeply interconnected. Insulin resistance and oestrogen metabolism interact. Adipose tissue produces oestrogen, so significant changes in body composition have hormonal effects. Women in perimenopause have specific metabolic vulnerabilities that affect how they respond to these medications.

This doesn't mean GLP-1 medications aren't appropriate for perimenopausal or menopausal women. For some women, they're a genuinely useful clinical tool in the context of significant metabolic risk. But it does mean that a prescriber who understands both metabolic and hormonal health is better placed to support you than one who doesn't.

What to expect if you've been prescribed one

The first four to eight weeks often involve side effects. Nausea is the most common. It typically reduces over time as the dose is increased gradually. Eating smaller portions, avoiding high-fat meals especially early in treatment, and staying hydrated all help.

The appetite reduction tends to be most noticeable in the first few months. Some women find it levels off over time. This is normal and doesn't mean the medication has stopped working.

If you feel unsupported, ask for more support. You should have a prescriber who is monitoring your response, reviewing your progress, and adjusting the approach based on how you're doing. If you don't have that, it's worth seeking it.

If you're researching before starting

Talk to a prescriber who can take a full history and assess whether this is appropriate for your individual situation.

Ask what monitoring and support will be in place.

Think about what changes you're willing to make to nutrition and movement alongside the medication, because that combination is where the outcomes are best.

And be honest with yourself about your expectations. These medications can be genuinely effective tools for metabolic health. They're not a substitute for addressing the underlying patterns.
