You've done the diets. You know what to eat. The weight still won't shift, and the hunger never switches off.

You're tired of starting again on Monday. Tired of being told to eat less and move more, as if you hadn't thought of that.

Weight is rarely about willpower. Hormones, blood sugar, sleep, stress, your gut and the medicines you take all push on it. We find out which ones are pushing on yours, and treat those.

Is this you?

Nod at a few of these and this consultation is for you.

Constant hunger

Food is on your mind all day, and you're never quite full.

Weight on the middle

It's settled round your tummy, even though you eat the same as before.

Tried everything

Diets work for a few weeks, then the weight comes back.

Bloated and uncomfortable

Bloating, reflux or constipation most days, and you can't tell what sets it off.

Energy crashes

A slump after meals, or by mid-afternoon you're running on biscuits and coffee.

Hormones changing

Perimenopause, menopause or low testosterone, and your body has changed with it.

How it works

Where it's safe and right for you, treatment can start after the first appointment. You don't wait weeks for bloods before anything happens.

1

2

Start

If a medicine is right for you, we decide together and it starts at a low dose, alongside your eating and strength plan.

3

Test

The blood tests that help find what's driving your weight: thyroid, blood sugar, iron, vitamins, hormones, cholesterol and liver.

4

Review

Your weight, side effects, energy and results, with the dose and the plan adjusted as you go.

Medical weight loss

If a weight-loss medicine is right for you, it sits alongside food, movement and a way of living that holds once the medicine stops. Each one is prescribed only if it is suitable for you.

Mounjaro (tirzepatide)

A weekly injection under the skin, using a pen. It works on 2 gut hormones, GIP and GLP-1.

Wegovy (semaglutide)

A weekly injection under the skin, using a pen. It works on 1 gut hormone, GLP-1.

Foundayo (orforglipron)

A tablet once a day, for people who would rather not inject. It works on GLP-1. Licensed in the UK in August 2026.

How they work

You feel full sooner and stay full for longer. Hunger and food noise quieten, your stomach empties more slowly, and your blood sugar is steadier after meals.

Side effects, planned for

Nausea, constipation and reflux are the common ones, mostly early on and after a dose increase. We go up slowly, and your eating plan is built around them: smaller meals, protein first, fluids and fibre, and less fat late in the day.

Which one, and whether at all

It depends on your history, your goals and how you get on. If a medicine is right for you, we decide together. If it isn't, the rest of the plan still works on your weight.

Alongside the medicine

Muscle and skin are protected in every weight plan, by default. Losing weight fast without protecting them is how people end up weaker, more tired and looking drawn.

Protein first

Protein at every meal, eaten first, so you keep muscle while you eat less. Your plan gives you real meals you'll actually cook.

Strength training

Lifting, resistance bands or bodyweight work, at least 2 days a week. Muscle keeps you strong and keeps your metabolism working for you.

Sleep and stress

Short sleep and constant stress turn hunger up and make weight harder to lose. We look at both and pick what you can actually change.

Alcohol

It adds calories, disturbs sleep and can make nausea worse on treatment. We agree what works for you.

Hormones and weight

Perimenopause and menopause

As oestrogen falls, many women gain weight round the middle and sleep worse. For some, body-identical HRT is part of the plan. See the Menopause Service

Testosterone

Low testosterone and extra weight often travel together in men, and low levels affect energy and strength in women too. For some men and women, body-identical testosterone is part of the plan. See Testosterone Therapy

Your gut, and finding the culprits

Bloating, reflux, constipation and IBS-type symptoms often sit alongside weight, and some weight-loss medicines make them worse at first. We find what sets yours off.

The triggers

Certain foods, stress, poor sleep, some medicines, hormones and the stage of your cycle can all set the gut off. A food and symptom diary shows which ones are yours.

One change at a time

We change one thing, watch what happens, then change the next. That way you know what actually helped.

The tests that help

Iron, B12, vitamin D and inflammation in the blood, and where your symptoms point to coeliac disease, the right test before you cut out gluten.

Questions people ask me

About the appointment
Is this for men too?

Yes. The Weight Management Service is for women and men. Men who also have symptoms of low testosterone can ask about it in the same consultation, and the Testosterone Therapy page explains that side.

Is it video or phone?

Either. You choose video or voice call, from anywhere in the UK.

Do I need a referral?

No. You book online yourself.

Do I need a certain BMI?

For the weight-loss medicines, the UK licences use a BMI of 30 or more, or 27 or more with a weight-related condition such as high blood pressure or type 2 diabetes, and UK guidance lowers these by 2.5 for people of South Asian, Chinese, other Asian, Middle Eastern, Black African or African-Caribbean family background. Your height and weight are checked at the consultation, and if a medicine isn't the right route, the rest of the plan still is.

I'm already on a weight-loss medicine from somewhere else. Can I switch to you?

Yes. Bring the name, dose, your start date, your starting weight and any side effects. We check it all at the consultation and decide together whether to carry on, change the dose or change the medicine.

Will my GP know?

If your GP needs to know about your treatment, I write to them, so your records match.

Treatment
How do these medicines work?

They copy gut hormones your body already makes after a meal. You feel full sooner and for longer, hunger and food noise quieten, your stomach empties more slowly, and your blood sugar is steadier. Mounjaro works on 2 of those hormones; Wegovy and Foundayo work on 1.

How much weight will I lose?

Nobody can promise a number, and results vary a lot from person to person. What's realistic for you, with or without a medicine, is something we work out together at the consultation.

What are the common side effects?

Nausea, constipation, reflux, diarrhoea and feeling full too quickly, mostly in the first weeks and after each dose increase. We go up slowly, adjust the dose, and give you an eating plan built around them, so most people settle.

What happens when I stop?

Appetite usually comes back when a weight-loss medicine stops, and weight often follows. That's why we plan the stop from the start: strength, protein, habits, and a slow step down if that suits you.

Will I lose muscle?

Some of any weight lost is muscle, which is why protecting muscle is in every plan by default: protein first at every meal, and strength training at least 2 days a week.

Can HRT or testosterone help with my weight?

Neither is a weight-loss medicine. For some women in perimenopause or menopause, body-identical HRT helps sleep, energy and mood, which makes the rest of the plan easier to keep, and some men and women with low testosterone feel the same. If that fits you, we decide it together.

Do I need a blood test before I start?

Not always. Where it's safe, treatment can start after the consultation and the bloods follow, so you're not waiting weeks to begin. If you already have recent results, bring them.

Gut health
Can you help with bloating, reflux and IBS-type symptoms?

Yes. We look for the triggers: certain foods, stress, sleep, medicines, hormones and your cycle. Then we change one thing at a time, with the blood tests that rule out the things that shouldn't be missed.

Should I do a food intolerance test?

The IgG food intolerance tests sold online aren't a reliable way to find triggers, so we don't use them. A food and symptom diary, a guided change in what you eat, and the right blood tests tell you more. If your reactions sound like a true allergy, an allergy blood test can help.

Medicines, delivery and urgent symptoms
How is my medicine delivered?

If we agree on a treatment, it's posted to your home through Healistic, our home delivery partner.

How do I get repeat prescriptions?

Once you've started at the clinic, you request a repeat online. Your reviews stay on their schedule, because the dose and the plan follow how you're doing.

Who isn't it suitable for?

These medicines aren't used in pregnancy or breastfeeding, or when you're trying to conceive. A history of pancreatitis, a personal or family history of medullary thyroid cancer or MEN2, type 1 diabetes, severe gut problems, an operation coming up, or a past or current eating disorder needs extra care and changes the plan, which we go through at the consultation.

I take the contraceptive pill. Does that matter?

Yes. Mounjaro can make the pill less reliable, especially in the 4 weeks after starting and after each dose increase, so you'd use condoms as well in those weeks, or use a coil or implant, which aren't affected. Vomiting or diarrhoea can make any pill less reliable too. If you're planning a pregnancy, tell us early: depending on the medicine, it's stopped 1 to 2 months before you try.

What symptoms need urgent help?

Severe tummy pain that doesn't go away, often going through to your back, with or without vomiting, can be pancreatitis: stop the medicine and call 999 or go to A&E. Pain under your right ribs with a fever, or yellow skin or eyes, can be your gallbladder: get urgent care the same day. Vomiting with a swollen tummy and no poo or wind needs A&E. Vomiting you can't keep fluids down with, very little urine or dizziness needs 111 the same day. Vomiting that keeps coming back: tell the clinic the same day so the dose can change. Blood in your poo, weight loss beyond what you're aiming for, difficulty swallowing or a lump in your tummy needs an urgent referral through your GP, within 2 weeks: ask for it by name. If you're under the clinic's care, the clinic writes to your GP the same day.
